STUDENT WITHDRAWALS:Families who need to withdraw their student from school, may now submit their form electronically. Verification of enrollment into the new school must be provided within 10 SCHOOL DAYS.  |  WITHDRAW FROM CMS

HUSKIE PANTRY: Help us fill our Huskie Pantry! Donation bins are available in all school offices and the BOE. Bins will also be out at all school-sponsored events.  |  HUSKIE PANTRY

IMMUNIZATION INFORMATION: All immunization records were due by Friday, September 20, 2019. 7th grade students must have a TDAP and meningococcal vaccination and 12th graders must have a second meningococcal booster. The Geauga County Health Department offers clinics in Middlefield and surrounding areas for those who may need it.  | Immunization Summary

FOOD SERVICE PROGRAM:  Cardinal Local Schools knows that a healthy meal helps fuel students for better learning opportunities.  Applications for Free/Reduced meals may be completed at any time throughout the school year.  A new application is required every year.  Please direct your application request to the CMS office at 440/632-0261 ext. 4001  Mrs. Tally Hostetler.

FREE MEALS UNTIL DECEMBER 31: We are pleased to share with you that the U.S. Department of Agriculture (USDA) is extending its summer meal program through the end of the calendar year.  This extension will allow the Cardinal Local School District to provide FREE breakfast and lunch to each student during school days. Re-Opening Plans 2020-2021 


DISTRICT-WIDE REMOTE LEARNING MEAL REQUEST: Should the district move back to all remote learning due to COVID-19 reasons, whether it’s for one day or several, our food service department will continue to provide meals on those days. We need to prepare for this now and are asking families to please review the information provided here and submit the form if you’d like to participate in this service.  |  INFORMATION  |  MEAL SIGN-UP

TRANSPORTATION OPT OUT: Parents who wish to transport their child(ren) to school this year in lieu of riding the bus should complete this transportation opt-out form. Students riding the bus will sit two to a seat and be required to wear masks at all times. We can fit a maximum of 44 students on each bus.  You can opt back in for transportation after opting out, however, this form should only be used by those who plan to transport their child(ren) the majority of the time; this is NOT to be used for daily change requests.  |  OPT-OUT FORM

RE-OPENING PLANS Q & A – Please take a moment to read through the plans carefully. We have done our best to answer as many questions as we could at this point. Not all details are finalized currently and we appreciate your patience as we continue to work through the challenges. Please also note that this is a working document and the plans could change depending on mandates, recommendations, and the current pandemic situation.  The deadline to enroll in at-home learning has also been extended to August 7. Thank you!


Governor Mike DeWine announces schools can reopen in the fall with precautions. Guidelines include assessing for symptoms, hand washing, sanitation and deep cleaning, social distancing, face coverings: mandatory for staff, strongly recommended for students in grades 3-12.  |  STATE GUIDELINES

DISTRICT WITHDRAWALS: Families who need to withdraw their students from the district may now submit their withdrawal form electronically. Please submit one form per student. Please note, if verification of enrollment into the new school is not received within 10 SCHOOL DAYS, the district is required to notify the Bureau of Motor Vehicles and the Geauga County Juvenile Judge. PANDEMIC ELECTRONIC BENEFITS TRANSFER (P-EBT) UPDATE: Ohio has been approved to operate Pandemic Electronic Benefit Transfer (P-EBT). Administered by the Ohio Department of Jobs and Family Services, the P-EBT program provides additional monetary assistance to households with children eligible for free or reduced-price meals dealing with school closures.  | FLYER
