Note from the Nurse

Parents of 6th graders moving into 7th grade, the state of Ohio requires that the Tdap vaccine be administered on or after your student’s 10th birthday and a dose of Meningococcal (MCV4) be administered prior to entering the 7th grade.  These vaccines may be offered at your doctor’s office, Rite Aid, CVS, Walgreens, or Wal-Mart.  The Geauga Health Department may be able to provide assistance if needed.
The school health staff is required to complete vision and hearing screenings on all new students and all students in the 5th and 7th grades, unless their parent declined the screenings.  These screenings should begin in the middle of October.  If you have concerns about your child’s vision or hearing and they are not in these grades, please contact me to discuss options.  I can be reached at 440-632-0261, ext 4005 or marie.roake@cardinalschools.org.  If your student fails a screening, a re-screening will be completed about a week after the first screening.  If they fail the re-screening a letter will be sent home explaining which screening was failed and why they failed it.  Please take the letter to a doctor and have it completed.  Please return completed forms to the Middle school or have it faxed to 440-632-0294.
Thank you.
