Important Health Paperwork:
Annual Health History Form– It is important to complete this form every year so that staff is aware of any medical changes or concerns for your student. If your student has any health changes please send in paperwork from their doctor(s) for our records. If your student needs this form completed, it can be found through the link listed below.
Immunizations- Students must be up-to-date on vaccines, have an exemption form turned in, or be in the process of getting up-to-date on vaccines by the 15th day of school or they will be excluded from school per Ohio law. Any 6th graders going into 7th grade need to have one dose of the meningococcal (MCV4) and one dose of the Tdap vaccines before the start of school. Any 11th graders going into 12th grade need to have a second dose of the meningococcal (MCV4) vaccine. The schedule of immunizations required for school by the ODH and the exemption form can be found through the links on the website.
Medication Administration- If your student needs medication, prescription or over the counter, during the school day please have their doctor fill out the Medication Administration Record and return it to school. If your student needs an inhaler or epi pen in school they can self-carry if the necessary paperwork is completed. Any medication needs to be brought into the office in a new, unopened container by a Parent or Guardian. The necessary paperwork can be found through the links listed below.
USDA Medical Statement- If your student has a food allergy this form needs to be completed by their doctor and returned so that we are able to accommodate any nutritional substitutions that are required. If your student needs this form completed, it can be found through the link listed below.
Action Plans– These forms need to be completed by your student’s doctor if they have asthma, a seizure disorder, diabetes, migraines, an allergy to bee stings, or an allergy that may require them to need an epi pen. We ask that these forms be completed even if your student does not need medication at school so that we know how to care for your student in the event of a medical emergency. Screenings- The state of Ohio mandates that school health staff need to complete vision and hearing screenings for students in Kindergarten, 1st, 3rd, 5th, 7th, 9th, and 11th grades and all new students. If your student sees an ophthalmologist or their primary care physician and has these screenings completed please send the paperwork into the school health staff. Please complete and return either the permission slip or waiver form at the beginning of the school year.
